Life feels like you’re always one step behind, always slightly on fire, always waiting to be found out.

For a surprising number of people, the missing piece is ADHD or Attention Deficit/Hyperactivity Disorder.

When most people picture ADHD, they picture a kid who can’t sit still. What they don’t picture is the 34-year-old woman who has held down a job for years through sheer force of will but cries in her car every evening from exhaustion. Or the guy who has been called “too sensitive” his whole life and can’t figure out why his relationships keep blowing up. Or the person who has been treated for anxiety and depression for a decade but still feels like they’re managing symptoms rather than actually getting better.

ADHD is a neurological difference in how the brain regulates attention, emotion, and motivation. Here’s the short version of what’s happening up there: the ADHD brain has a dopamine regulation problem. Dopamine is the neurotransmitter most associated with motivation, reward, and follow-through. In a neurotypical brain, the promise of a future reward- finishing a project, paying a bill on time, having a calm conversation- is enough to activate the brain’s motivation system. In the ADHD brain, that future reward often doesn’t register strongly enough to get things moving. What the brain responds to instead is right now– urgency, novelty, passion, or crisis.

This isn’t laziness. It isn’t a bad attitude. It’s a brain that is genuinely wired differently, and it shapes nearly everything.

Once you understand the dopamine piece, a lot of “self-destructive” ADHD behaviors start to make a different kind of sense.

Take procrastination. For most people with ADHD, the stress and cortisol spike of almost missing a deadline is the actual fuel that gets the work done. The panic is not incidental- it’s functional. It generates the neurological activation that the brain couldn’t manufacture from planning ahead. This works, until it doesn’t. Until the deadlines are too big, the stakes too high, or the body too burned out to keep running on cortisol.

Or take conflict in relationships. This one is harder to talk about, but it’s important: sometimes picking a fight is dopamine-seeking behavior. Emotional intensity- even *negative* intensity- activates the ADHD brain in a way that calm, routine connection doesn’t. This doesn’t mean people with ADHD want to hurt the people they love. It means their nervous system is looking for stimulation, and conflict delivers it fast. Understanding this pattern is often the beginning of being able to change it.

One of the most underrecognized features of ADHD is rejection sensitive dysphoria (RSD)- an intense, often overwhelming emotional response to the perception of criticism, failure, or rejection. And note that word: perception. You don’t have to actually be rejected. You just have to think you might be.

For people with rejection sensitivity, a friend’s short text response can feel like abandonment. A neutral expression on a partner’s face can feel like anger. Constructive feedback at work can feel like confirmation that you’re fundamentally not good enough. The emotional experience is real and intense- it’s not drama, it’s not manipulation, it’s neurological- but it can make navigating relationships exhausting and sometimes explosive.

If you’ve spent your life assuming people are mad at you, reading rooms obsessively, preemptively apologizing, or pulling away from people before they can leave you- rejection sensitivity might be part of your story.

Here’s what we want you to hear: understanding that you have ADHD doesn’t excuse the impact your patterns have on yourself and others. And it also means you didn’t choose this, you’ve been doing your best, and there is real room for things to get better. Both of those things are completely true at the same time.

You can acknowledge that you made a mistake, AND acknowledge that you didn’t mean to. You can take responsibility without collapsing into the story that you’re a terrible person. That kind of self-compassion isn’t letting yourself off the hook. It’s the foundation you need to actually change.

Treatment for ADHD is not just one kind of treatment- a combination tends to work best. Medication, when appropriate, can be genuinely life-changing by addressing the underlying dopamine regulation issue directly. Mindfulness and somatic practices help build the self-awareness to catch your patterns before they run the show. And therapy- especially with someone who understands neurodivergence- can help you develop new ways of relating to yourself and others without the chaos.

Over the past several years, the mental health industry has attracted enormous amounts of outside investment. Independent practices that were once owned by a local clinician get acquired and absorbed into larger networks. Insurance companies have begun investing in or partnering with the very therapy platforms they reimburse, creating a cozy arrangement that raises real questions about whose interests are actually being served.

We want to be clear- we’re not saying everyone who works at or uses these platforms is doing something wrong. Many good therapists work within these systems because they have to, and many clients find them because they don’t know there’s another option. What we are saying is that the forces reshaping the mental health landscape right now are primarily driven by profit, not care. And that matters.

There’s a reason therapy has traditionally been a deeply local, deeply human practice. Healing happens in relationship. It happens in the particular way your therapist remembers what you said three sessions ago, or notices something shift in your body before you’ve found the words, or stays curious about you as a whole person rather than a set of symptoms to be managed.

That kind of care is hard to scale. And scale is exactly what large, investor-backed platforms are trying to achieve.

One concern we hear more and more in our field is the push- quiet for now, but real- toward replacing human therapists with AI-driven tools. Chatbots. Automated check-ins. Algorithmically generated coping suggestions. We want to name this plainly: we don’t believe an AI can provide therapy. We believe therapy is a fundamentally human exchange, and that any system that positions automated tools as a substitute for a real therapeutic relationship is cutting corners in ways that can genuinely harm people. This isn’t technophobia. It’s a values statement. Humans deserve to be cared for by other humans, especially when they’re vulnerable.

When a locally owned therapy practice closes or gets absorbed into a larger network, the community loses something specific and hard to replace.

Local therapists are embedded in the communities they serve. We know Tucson. We know its particular stressors- the heat, the border politics, the housing crunch, the tension between rapid growth and a city that wants to *stay weird*. We have relationships with other local providers, with community organizations, with the social and cultural fabric of this place. That context matters clinically. A therapist at a national platform handling hundreds of clients across five states doesn’t have it, and can’t.

Local practices also tend to be more accountable. If something goes wrong, or if your needs aren’t being met, you can have a real conversation with a real person who has a stake in this community. That accountability disappears inside a large corporate structure or when admin is outsourced to AI or virtual assistants on the other side of the world.

And economically: when you pay a locally owned practice, that money largely stays here. It pays local salaries, supports local families, and gets spent in local businesses. When you pay a venture-capital-backed platform, a significant portion of that money leaves Tucson entirely.

So what can you actually do? Here are some concrete steps:

Ask who owns the practice. It’s a completely reasonable question. Is this a locally owned business, or is it part of a larger network or platform? Independent practices will usually be proud to tell you.

Be skeptical of heavy advertising. Local therapists generally don’t have marketing budgets. If you’re seeing a lot of polished ads for a therapy service on social media, it’s worth asking how they’re funding that- and what they expect in return.

Check Psychology Today and TherapyDen. Both directories let you filter for independent practitioners. TherapyDen in particular has a strong emphasis on values-aligned, social-justice-oriented therapists if that matters to you.

Ask about insurance arrangements. Some large platforms have exclusive or preferred relationships with certain insurers. A local therapist who takes your insurance independently is a different arrangement than one whose practice is effectively co-owned by the company reimbursing them.

Refer your people to local therapists. Word of mouth is how independent practices survive. If you’ve had a good experience with a local therapist, tell someone. That referral is genuinely valuable in a way that liking a social media post is not.

If cost is a barrier, ask directly. Many local therapists offer sliding scale fees. We would rather work with you at a reduced rate than lose you to a platform that doesn’t actually have your best interests at heart. The conversation is always worth having.

We’re living in a moment when consolidation of power and money is happening everywhere- in healthcare, in media, in housing, in retail. Small, human-scale institutions are being steadily replaced by large ones optimized for shareholder returns. Therapy is not immune to this, and neither is Tucson.

Choosing a local therapist is a small act, but it’s a meaningful one. It’s a vote for the idea that care should be provided by people who are genuinely invested in your wellbeing and in the health of this community not by platforms that see your mental health as a market to be captured.

We’re here because we love this work and we love this city. We’d be honored to be your choice for such personal, healing work.

The human experience is impacted greatly by the things happening around us. Medication is not meant to be a bandaid to the concerns. It is meant to be a tool that supports lifting you up, to allow you the space to process and grow. This is also where therapy comes in! A combination of talk therapy and medication can improve outcomes and lower changes of recurrent symptoms.
